Below you will find several practical forms that our team needs in order to keep preparing to host you throughout the 18 Inch Journey. Please complete the following forms no later than February 3rd. We recognize that you shared some of this information on your application, however we know medical information and food restrictions can change over time. We’re asking you to answer every question, even if your information hasn’t changed. In the event that you have any medical or dietary-related updates,  please email our team.

After you submit a form, you’ll see a page that confirms it was received by our team. From there, you can return to the Practicals page or Home page. You will see a “Complete and Continue” button at the bottom of every form. Clicking this button allows you to see the next form, but does not submit your form to our team. You must click “send” for your form to be processed.

Please note that you will need to keep track of your own progress as you complete your forms.  This webpage will not keep track of your submissions for you.
